Scheduling an Appointment with a Neurologist

If you are feel like you are having signs and symptoms of neuropathy, first ask your primary doctor if seeing a neurologist would be beneficial in your overall treatment plan.

As a new patient, your primary doctor can either send a written referral or provide a verbal referral to our center in order for you to see Dr. McCoy. NOTE: Dr. McCoy is performing EMG / Nerve Conduction Tests only.

If you have already been to a neurologist and would like us to review your records to see if a 2nd opinion is appropriate or necessary, please call us at (501) 993-3715.

Patient Information Forms

For your appointment, you will need to complete important patient forms in order for us to have accurate patient and medical records. To save time during your appointment you can print them out and complete them prior to coming to the center.

Appointment Reminders and Arrival Times

New Patients Arrival Time:

  • If you completed the forms from the website: arrive 10 minutes prior to your scheduled appointment time.
  • If you need to fill out forms when you come: arrive 20 minutes prior to your scheduled appointment time.

Follow up Patients Arrival Time

  • Arrive 10 minutes prior to your scheduled appointment time.

If you are scheduled for an EMG Test please do not use body lotion on the day of the test and please shower to remove oil from your skin.

Bring your updated insurance card when you come for your appointment.

Cancelling an Appointment

If you need to cancel an appointment, please call the office at least 24 hours in advance of your scheduled appointment at (501) 993-3715.

Note: If you do not call at least 24 hours in advance you will be charged a $50.00 cancellation fee.
